As a mom myself I’m happy to admit that I love anything handmade or DIY that my kids have created and over the years I’ve definitely kept a bit of a stash of the things my kids have given me on this special day.

How To Use Your Mother’s Day Questionnaire For Kids

Start by printing the digital file onto white paper or cardstock. Normal printer paper is absolutely fine for this but if you want to make this a bit more durable and use this as a Mother’s Day gift then I’d opt for cardstock instead. Another option could be to put the finished questionnaire in a picture frame.

There are five options of different designs in the PDF file for you to choose from with mom, mum, mommy, mummy and a blank option that you could use for step-parents, grandma or whoever your kid would like to show some love and appreciation for. Just pick the one that’s right for your child.
